Dear Sir and Brother: Your remittance for assessment No 315 is re-
ceived and placed to your credit. If received after Nov. 30, 1896, it is
subject to the following:
If this remittance is not received by the Treasurer within two calendar
months from Oct. 1, 1896, membership in the Benefit Department ane all
right to any benefit therein is forfeited. It it is accepted by the Treasurer when
received after Nov. 30, 1896, it is subject to the condition that the certifi-
cate holder is in good health and free from injury, and when so accepted, all
rights to any benefits for death or disability resulting from any sickness or
injury existing at, or prior to the date of its acceptance by the Treasurer as
shown by his books, is forfeited.
This receipt is not valid unless dated. Balance to Credit $.
