The miniature chest x-ray film taken of the person whose name appears on the reverse side of this card showed no evidence of active tuberculosis on the above date. This card when stamped with the seal of the Health and Hospital Corporation of Marion County, may be presented by food personnel, baby sitters, domestics, barbers, beauticians, and others as official evidence of having had a chest x-ray. This card good for one year. However, a good report today is no guarantee for the future. A physical examination, by your physician, including a chest x-ray is recommended at least once a year as a check for TB, heart, lung cancer, and other chest diseases.
